INSTRUCTIONS

This order form is here for you to print out on your printer and fill out manually. Upon completing the order form you may either fax or mail it (mail usually takes 10 days to receive when mailing from the U.S.).

To print out this order form, first ensure that your printer is connected and turned on. Then click on the words below that say GO TO MANUAL ORDER FORM after reading the remaining instructions.

Once you are on the manual order form page do the following. Using your mouse, move to the top of the screen to where the FILE menu is and left click once. In the FILE menu, select PRINT by left clicking on it with your mouse. The order form should then begin printing out on your printer.
